
Care Coordinator Introductions
The Montana Health Network Health Insurance Plan and Trust has added care coordination to the benefits offered to our member facilities and their employees. In their roles as Care Coordinators, Rebecca Stensvad, RN, and Charlee Allison, RN, will review members’ health records and help fill care gaps. They will monitor members’ health and wellness by reviewing insurance claims and providing feedback and suggestions for ongoing processes. They will also help with any questions about referrals, procedures, and the billing processes as well as helping with transitions of care should there be a need for that. Care Coordinators will send out reminders and help schedule appointments as needed. They will periodically do medication reviews to ensure members are maximizing medication potential and answer questions members may have. They will also act as patient advocates and relay information to members and their primary providers.
Rebecca was born and raised on a farm/ranch in Eastern Montana and has worked in rural healthcare for over 18 years. She owns her own business, the Melstone Mercantile store in Melstone, Montana. Rebecca graduated from MSU-Bozeman with a Bachelor of Science in Nursing in 2005. She has worked in the inpatient setting, inpatient cancer care, ICU, telemetry, long term care, and was most recently the Clinic Manager in a small rural facility. In that role, Rebecca created and implemented the Chronic Care Management (CCM) and Integrated Behavioral Health (IBH) Programs. She has worked in collaboration with many rural Montana groups to successfully coordinate care for patients and transition them to appropriate levels of care. Rebecca believes in, “an ounce of prevention,” and understands that progressive forward thinking is driving our healthcare systems in this day in age. Charlee was born and raised in Eastern Montana and has been working as an RN for over 10 years. She graduated in 2013 from Miles Community College with an Associates of Science in Nursing. Charlee has worked in both rural and urban areas, including medical inpatient, long term care, hospice, home health, and clinic settings. She has been working as a patient care coordinator since 2020 and is currently working for Montana Health Network as a Regional Care Coordinator focused on value-based care delivery. Charlee’s “why” in nursing has always been patient advocacy. She will offer patient centered care that promotes confidence, education, and autonomy in members’ health choices.
